Caledonia Home Health and Hospice is seeking a compassionate nurse to join their team as a Hospice Care Coordinator. This position is responsible for coordinating hospice services for clients and their families, managing hospice supply and medication processes, and establishing and maintaining positive relationships with referral sources. The Hospice Care Coordinator is an integral part of the interdisciplinary care team responsible for developing and managing hospice care plans. Must possess a current Vermont Registered Nurse license and at least three years of experience in health care coordination preferably in hospice or similar. The successful candidate will be able to demonstrate good communication, written and verbal, and the ability to foster strong community relationships.
